How do I access IFMG for any of the medical services provided?
You must have an appointment to access IFMG. We do not accept walk-ups to the medical office.
Does IFMG also operate as an Emergency Room/Urgent Care?
No, IFMG functions as a primary care medical office. For any emergency or urgent health situation, call 911 or go to your local hospital emergency room.
How do I make an appointment?

Who is medical care provided by?
Comprehensive care will be delivered by a team of providers that include an MD, psychiatrist, advanced practice nurse, counselors, and behavioral health therapists.
What insurance do you take/What if I don’t have insurance?
IFMG accepts patients with Medi-Cal, Medicare, and CalOptima Community Care Network. In partnership with Illumination Foundation, we also provide services to those individuals who are receiving housing assistance from any Illumination Foundation site or emergency shelter.
Is IFMG only located at the Fullerton Navigation Center?
IFMG's primary care clinic is located on the second floor of the Fullerton Navigation Center, 3535 W. Commonwealth Ave, Fullerton, CA 92833.
Can I schedule an appointment for my friends or family members?
Illumination Foundation Medical Group accepts self-referred patients as well as referrals from other individuals, families, community groups, and healthcare organizations. We are Medi-Cal and Medicare-approved providers and have a contract with CalOptima to ensure everyone has a chance to access our services. You do not need to have a current primary care physician.
